IMPORTANT INFORMATION - READ CAREFULLY

CERTIFICATION AND AUTHORIZATION: I hereby certify that the statements contained herein are true and correct to the best of my knowledge. I understand that should an investigation disclose material misrepresentation, omissions or falsification my application may be rejected -or if a member of SVFA my membership and all rights and privileges of my SVFA membership may be immediately terminated. By signing this form, I authorize review and full disclosure of all records, or any part thereof, concerning myself by and to the membership committee and the chief of the SVFA, whether said records are of public, private or confidential nature. I hereby authorize release to the SVFA membership committee, and to the Fire Chief of the Southbury Volunteer Firemen’s Association any information or records whether criminal, disciplinary, a conviction, or motor vehicle violation.
Further, I hereby make my application for membership in the Southbury Volunteer Firemen’s Association Inc., and if accepted I agree to:
1) Take and pass a medical physical exam and drug test/screen.
2) Abide by all the rules, by-laws, procedures, and regulations of the SVFA.
3) Attend required training, and maintain my readiness.
4) Attend social functions, as possible.
